What documents are required when applying through an agent?

When an agent applies, the agent's identity verification documents and required documents will be necessary.

Please check the following for details on the documents required for the procedure.

Documents required if the visitor is other than a legal guardian

Documents required for device upgrade

  • If the contract holder is a minor and the agent is a facility-related person, a certificate verifying that the agent is a facility staff member is also required.

Documents required for cancellation

  • If the contract holder is a minor and the agent is a facility-related person, a certificate verifying that the agent is a facility staff member is also required.
  • If the contract holder is missing, a certificate confirming the missing status is also required.
  • If the contract holder is an elderly person and the agent is a lifelong support service provider for the elderly, a certificate verifying that the agent is an employee of the provider is also required. In addition, a power of attorney is required even in the event of the contract holder's death, so it is necessary to prepare a power of attorney in advance, while the contract holder is still alive.

Documents required for changing contract information

(Change of name or surname, address, contact phone number, current plan, optional services and discounts, change of user information)

  • Depending on the procedure, additional documents may be required.

Documents required for new contract or MNP

(Only if the contract holder is eligible for the Smile Heart discount)

  • If the contract holder is a minor and the agent is a facility-related person, a certificate verifying that the agent is a facility staff member is also required.

  • Depending on the procedure, additional documents may be required.

Documents required for new contract or MNP

  • Identity verification documents of the agent
  • Certificate of Registered Matters (issued within the last 3 months) certifying adult (statutory or voluntary) guardianship
  • Set the payment method to one of the following

    In the case of credit card withdrawal: Credit card (including debit card)

    In the case of direct debit from bank account: Cash card or bank passbook + registered seal with the financial institution

  • MNP reservation number (only for new MNP contracts)

For your reference

  • If the contract holder is a minor and the visitor is the guardian with parental authority, the following documents will be accepted in place of a power of attorney from the contract holder.

    In the case of a guardian with parental authority who has already been registered with our company: the guardian with parental authority's identity verification documents

    In the case of a guardian with parental authority who has not been registered with our company: documents that prove the parent-child relationship (Certificate of Residence, Certificate of Eligibility)

    • However, if the procedure for new contract, new MNP contract, device upgrade involves an application that requires the consent of the contract holder, a power of attorney from the contract holder is required.
  • All of the indicated amounts exclude tax unless otherwise specified.
